Dr. Fazeli is fellowship trained and Board Certified in Geriatric Medicine. He provides services to patients aged 65 and older in a unique practice based at the greater Portland area assisted living and nursing home facilities. Dr. Fazeli makes his visits on site without the need for office appointments. A community clinic in Falmouth opened in May of 2004 for independent elderly seeking outpatient Geriatric consultation. Dr. Fazeli also provides consultation services to patients at both Portland Hospitals and is active in the teaching of the new Generation Geriatricians through the new Maine Medical Center Geriatric Fellowship program.

Outpatient Geriatric Clinic:
Dr. Fazeli has an out patient Geriatric clinic at OceanView in Falmouth. Patients living independently are served through this Geriatric clinic. Patients are seen to address problems such as memory issues, depression, falls, complex medical problems, polypharmacy (too many medication), preliminary driving ability assessment, placement questions, frailty, incontinence, behavioral problems, non-healing wounds among others.
